1. Doctor may accept payment from the plan as full payment
2. Doctor may bill a voluntary agency which will bill the provincial plan which will pay the agency which will pay the doctor as full payment.
3. Doctor bills patient and supplies detailed statement. Patient submits bill and statement to plan which pays patient who pays doctor. Doctor can extra-bill.
4. Doctor bills patient without statement. The patient pays him. Plan pays nothing.
