| This file contains an optional flow-sheet that one of our faculty developed last year. Students or faculty may choose to use this sheet to keep track of family issues.The sample flow-sheet will help students map out their: 1) problem list, 2) team's prioritization/value of the problems, 3) team's goals, 4)patient involvement in prioritization of the problems and goals, 5)throw in some interdisciplinary awareness, 6) periodic (i.e., during each home visit) re-evaluation of the priorities of the problems and goals, 7) final assessment of how well they prioritized the problems, their ability to solve the problems, and achievement of the goals. |
