Forms for evaluating students:
Student Self-Assessment/Mid-term Feedback form - the student initiates, gives to attending for completion
Evaluator Form - evaluator completes at the end of the rotation, turns in to Course Director
Summative Feedback - the course director fills out the online Form for Elective Grades and Summative Evaluation of Competencies by Faculty (see 4th year home page)
