Cephalization is not seen in this CXR. Cephalization is the reversal of the normal vascular pattern in upright individuals (ie. the upper lobe vessels would be larger, more prominent than the lower lobe vessels). Cephalization is a manifestation of chronic left heart failure but it is not seen in acute heart failure, ie. Acute myocardial infarction leading to left heart failure leading to pulmonary edema.
