It kind of reminds me of my friend whose wife insists that every time he buys a new shirt, he has to figure out which of the shirts in his closet can go. And maybe that's not a bad idea when we prescribe medications.

Somebody in the healthcare team has to coordinate the medications being prescribed by other practitioners, and the primary care physician probably is the best suited to do that.
