The transthoracic echo, if completely normal, does not eliminate the possibility of myocardial ischemia, particularly if it was done when the patient was not having chest pain.
However, if the patient is having the typical symptoms that brought the patient to the hospital and the echocardiogram shows normal regional wall motion, it is unlikely that the patient's chest pain is due to myocardial ischemia and it is almost a certainty that it is not due to a large myocardial infarction.
