The sequence of events I have just outlined leads us to a number of logical and now well-substantiated conclusions.
- First, internal carotid artery stenosis is important not because the stenosis is hemodynamically significant, but only insofar as the atheromatous lesion causing the stenosis leads to the formation of clot.
- Second, while stroke corresponds to a discreet thromboembolic event, infarction is clearly an evolutionary process that proceeds at varying speeds, depending upon the adequacy of collateral blood flow.
