Treatment should occur in the emergency room and in at least an intermediate care unit, so that the patient can be carefully monitored for the development of signs of intracranial hemorrhage.

Standard of care does not demand that every hospital be able to provide rTPA treatment of acute stroke. It is far preferable that acute stroke patients simply not be triaged to hospitals not prepared to meet the demands of this treatment.
