For true urinary tract infections, the causative organisms are similar in ambulatory elderly and other ambulatory populations. However, in the nursing home population, bacteria similar to hospital flora, including Pseudomonas and Staphyloccus (with their resistance patterns) are common.

If a patient is symptomatic and has bacteriuria, it is important to gram stain the urine to determine appropriate antibiotic therapy.
