Fever and Infection in the Elderly -- Pneumonia

Bacterial causes of pneumonia include -- Pneumococcus, Staphylococcus, and Gram Negative bacteria in this population.

They also have an increased risk for Legionella pneumophilia pneumonia.

Recommended empiric therapy for pneumonia in the elderly includes a third generation cephalosporin.

However, you may need to add a macrolide for coverage if Legionella is suggested by clinical findings or if there is no improvement in the first few days of therapy.
