The pneumoccocus is the most frequent cause of bacterial pneumonia. Antibiotic resistance is becoming more common, especially in nursing home residents.

In this patient, the lack of response to ampicillan suggested either an antibiotic-resistant pneumoccocus or another organism such as a Gram-negative rod.

An inability to obtain sputum for culture is common in elderly patients
