In situations where a clinician is concerned about the management of penicillin-resistant pneumococcus and if the patient appears not to be responding, and I would say that this is a rarity, then one may consider using some of the newer quinolone antibiotics such as Tequin [gatifloxacin] or Levaquin [levofloxacin] or moxifloxacin [Avelox] which are new quinolone antibiotics that are effective against penicillin-resistant pneumococci.
Macrolide antibiotics such as azithromycin and clarithromycin are alternatives but their efficacy may not be as great as that of the quinolone antibiotics because there is also resistance to the macrolide antibiotics among the pneumococci.
